Science of Autoimmunity

Autoimmunity

Examples:

  • Type 1 Diabetes
  • Rheumatoid Arthritis
  • Systemic Lupus Erythematosus (SLE)
  • Multiple Sclerosis
  • Hashimoto’s thyroiditis
  • Graves’ disease
  • Myasthenia gravis
  • Celiac disease
  • Vitiligo
  • Some forms of autoimmune skin and blood disorders

Scientists have identified more than 80 autoimmune diseases, although their mechanisms और classification काफी diverse हैं।

Self →

Non-self → बाहरी/foreign substances

For example:

Immune Tolerance

Difference

Autoimmunity

Body के own components के against immune reactivity को broadly autoimmunity कहा जाता है।

Autoimmune Disease

Immune Tolerance:

  1. Central Tolerance
  2. Peripheral Tolerance

1. Central Tolerance

Central tolerance immune system की early education system जैसी है।

यह primarily:

  • Thymus — T cells
  • Bone marrow — B cells

Example

2. Peripheral Tolerance

Central tolerance perfect

Peripheral tolerance

  • Anergy
  • Deletion
  • Regulatory T cells
  • Inhibitory signals
  • Lack of appropriate activation signals

Regulatory T Cells — Immune System के “Brake”

Simple analogy:

Effector immune cells = Accelerator

Regulatory T cells = Brake

Genetic susceptibility + Environmental triggers + Immune dysregulation = Autoimmune disease risk

1. Genetic Factors

Having a risk gene does not automatically mean you will develop the disease.

2. Environmental Triggers

Possible factors

  • Certain infections
  • Smoking
  • Ultraviolet radiation
  • Certain chemicals/exposures
  • Some medications
  • Other environmental influences

For example, lupus में researchers genes, environmental exposure, immune and inflammatory factors के interaction का अध्ययन करते हैं।

3. Infections

Molecular Mimicry

Simple idea:

4. Smoking

5. Sunlight और UV Radiation

Sunlight generally life के लिए essential है।

For example, UV exposure और lupus activity के बीच relationship extensively studied है।

Sunlight ≠ automatically harmful

excessive UV exposure + susceptibility + specific disease mechanisms = possible problem

6. Hormonal and Sex-Related Factors

  • Hormones
  • Genetics
  • Immune regulation
  • X chromosome-related mechanisms
  • Environmental factors

Autoantibodies

Autoantibody

Antibody जो body’s own molecules या tissues के against react करती है।

  • ANA
  • Anti-dsDNA antibodies
  • Rheumatoid factor
  • Anti-CCP antibodies
  • Anti-TPO antibodies

B Cells

T Cells

T cells + B cells + antibodies + cytokines + innate immune cells

Cytokines

“Activate!”

“Come here!”

“Increase inflammation!”

“Stop!”

Types

1. Organ-Specific Autoimmune Diseases

Examples:

DiseaseMajor Target/Effect
Type 1 DiabetesPancreatic beta cells
Hashimoto’s thyroiditisThyroid
Graves’ diseaseThyroid stimulation
Myasthenia gravisNeuromuscular junction
VitiligoMelanocytes
Some forms of autoimmune hepatitisLiver

2. Systemic Autoimmune Diseases

Examples:

  • Systemic Lupus Erythematosus (SLE)
  • Rheumatoid Arthritis
  • Systemic Sclerosis
  • Sjögren’s disease
  • Some systemic vasculitides

For example, lupus कई organs including skin, joints, kidneys, lungs, heart और nervous system को affect कर सकता है।

Important Autoimmune Diseases

Type 1 Diabetes

Type 1 diabetes में immune system pancreatic beta cells को target करता है।

Beta cells insulin produce

Rheumatoid Arthritis

  • Pain
  • Swelling
  • Stiffness
  • Joint damage

Systemic Lupus Erythematosus — SLE

Lupus एक systemic autoimmune disease है।

यह:

  • Skin
  • Joints
  • Kidneys
  • Heart
  • Lungs
  • Brain
  • Blood

Multiple Sclerosis

  • Vision problems
  • Weakness
  • Sensory changes
  • Coordination problems

Hashimoto’s Thyroiditis

Over time thyroid hormone production

Graves’ Disease

Result:

Excess thyroid hormone production

Myasthenia Gravis

Result:

Muscle weakness

Inflammation

Normally:

Injury/Infection → Inflammation → Repair/Protection → Resolution

लेकिन autoimmune disease में:

Self-targeting immune response → Persistent inflammation → Tissue damage

Controlled inflammation = protection

Uncontrolled or inappropriate inflammation = potential tissue damage

No.

क्या Autoimmune Disease और Allergy Same हैं?

No.

AllergyAutoimmunity
Usually harmless external allergen के प्रति abnormal responseBody के own components के against immune response
Example: allergic rhinitisExample: type 1 diabetes
External allergen importantSelf-target important

क्या Autoimmune Disease और Immunodeficiency Same हैं?

Autoimmune disease

Immunodeficiency

Simple analogy:

Autoimmunity = Security system attacks the owner

Immunodeficiency = Security system is too weak to defend the building

Diagnosis disease-specific

Doctor generally consider

  • Medical history
  • Symptoms
  • Physical examination
  • Blood tests
  • Autoantibody tests
  • Imaging
  • Biopsy
  • Organ-specific tests

Genetics + Environment + Immune regulation + Unknown factors

  • Smoking avoid करना
  • Healthy lifestyle maintain करना
  • Appropriate vaccinations और preventive healthcare पर doctor की advice follow करना
  • Excessive UV exposure
  • Known disease triggers को understand करना
  • Regular medical care

Fascinating Research History

Around 1900 — “Horror Autotoxicus”

German immunologist Paul Ehrlich ने early 20th century में “horror autotoxicus” concept दिया।

इसका basic idea था कि normal body अपने against destructive immune reactions को रोकती है।

At that time, autoimmunity

Early Evidence

Research ने दिखाना शुरू किया कि antibodies कभी-कभी body’s own cells/components के against भी react कर सकती हैं।

1940s — Autoimmunity

  • Autoantibody research
  • Rheumatoid factor
  • LE cell phenomenon
  • Experimental thyroiditis

Conceptual Revolution

Scientific Acceptance

Modern Autoimmunity Research

Question 1

Question 2

Question 3

Question 4

Question 5

Question 6

Question 7

Interesting Fact: Autoimmune Diseases

Different autoimmune diseases में:

  • Different target tissues
  • Different immune cells
  • Different antibodies
  • Different cytokines
  • Different genetic risks
  • Different environmental triggers

Interesting Fact: Genes

“Genetic susceptibility may load the gun, but environmental factors may help pull the trigger.”

Antibody ≠ Automatically Disease

Central tolerance

Peripheral tolerance

Regulatory mechanisms

Activation checkpoints

Immune regulation

Students = Immune Cells

School ID Card = Self-recognition

Outsider = Pathogen

Security Guard = Immune System

Normally guard:

Student →

Unknown outsider →

Student → outsider

Simple Scientific Flowchart

Genetic Susceptibility

Environmental / Internal Trigger

Breakdown of Immune Tolerance

Autoreactive T & B Cells

Autoantibodies + Cellular Immune Responses

Inflammation

Tissue Damage

Autoimmune Disease

Quick Revision: Autoimmunity in One Page

Immune System → Normally protects the body

Immune Tolerance → Prevents attack on self

Central Tolerance → Thymus + Bone Marrow

Peripheral Tolerance → Controls self-reactive cells outside central organs

Tolerance Breakdown → Autoimmunity

Autoimmunity + Tissue Damage → Autoimmune Disease

Major Players → T cells + B cells + Autoantibodies + Cytokines + Innate Immune Cells

Major Risk Factors → Genetic susceptibility + Environmental influences + Immune dysregulation

Examples → Type 1 Diabetes, Rheumatoid Arthritis, SLE, Multiple Sclerosis, Hashimoto’s, Graves’ disease

Final Takeaway

जब self-tolerance की multiple protective mechanisms में disturbance होता है, तो autoreactive immune cells और antibodies body के own tissues को target कर सकते हैं।

A powerful immune system needs powerful regulation.
